Working daily to improve the health of our community
UniteWI trains and develops Community Health Workers (CHWs): trusted members of the communities they serve. Our CHWs use modern data tools to address the social determinants of health and improve health outcomes. Through community partnership, UniteWI is the bridge to optimal, efficient outcomes for community members and service providers alike.
CAARN: bridging communities and researchers for healthy aging
The Community-Academic Aging Research Network (CAARN) brings together community and health system partners with academic researchers to improve the health of older adults and reduce health disparities.
